APPLICATION FOR MEMBERSHIP
GENERAL INFORMATION

1400 Turk Hill Road
Fairport, New York 14450

Thank you for showing an interest in Perinton Volunteer Ambulance Corps (PVAC). The following application will start you on your way to helping us help the community.

Applicants for active membership must be at least 16 years old to Dispatch (Explorer Post members must be 15 years old), 18 years old to Medic, and 21 to Drive.

As a new member of PVAC, after your initial orientation, the applicant has the option to train as a dispatcher or to become a medic trainee. All new dispatcher trainees participate in a separate dispatch orientation. To be cleared as a Dispatcher, applicants must have completed all training requirements including CPR for the Professional Rescuer and Bloodborne Pathogens.

New applicants interested in medicing are required to observe two dispatch shifts to understand basic radio operation. In order to be cleared as a First Medic, the trainee must be certified in EMT - D (or higher), CPR for the Professional Rescuer, HAZMAT, AED and Bloodborne Pathogens and fulfill all training requirements.

To be eligible to train as a Driver, an applicant must be certified EMT, and a cleared first medic. All drivers must maintain all of their first medic responsibilities as well as CPR for the Professional Rescuer, HAZMAT, Bloodborne Pathogens, and attend an Emergency Vehicle Operations Course.

The above certifications are NOT required to become a member of PVAC. Applicants who do not have the appropriate certifications will receive free training and certification for any position desired. All active members, including those in training, must serve a minimum of 10 hours per month or 30 hours a quarter. Please note that this is a minimum requirement. We encourage members to work as much as possible, especially while training, to maintain and learn new skills.

All active members, including those in training, must wear the PVAC uniform at all times while on duty. The uniform shirt and pants will be provided after orientation. The uniform consists of a white button-down shirt with the appropriate PVAC patch, and navy blue pants. Uniforms must be neat, clean, and fit properly in order to give the professional image for which PVAC stands.

Additional information including copies of the By-laws, Rules and Regulations,
Dispatcher or Medic Manuals will be provided at orientation.
